Cost increases and utilization changes resulting in an increased cost of healthcare.  Unit cost increases include increased costs of services provided by hospitals and physicians.  Utilization changes refers to an increase or decrease in the use of services.  Additional cost drivers may include changes in plan benefits, demographic changes in the insured population, and legislative changes.
